After adding this to your cart-
Please specify in the notes area of the 'MY CART' page the SIZE, STYLE, COLOR, & DESIGN, of the shirt you want the last name placed on. PLEASE ALSO INCLUDE THE NAME. If you want a name on multiple items, you'll need to add it to the cart multiple times, once for EACH item.
***If there is nothing in the notes area, there will NOT be a name added as we cannot ensure which item you wanted this placed on. No refunds will be issued if your item doesn't get a name placed on it.
How the name is spelled in the notes is how it will be placed on the shirt, please double check!
